Food thickeners are covered by Oregon Medicaid when medically necessary and when documentation shows diagnosis of swallowing disorders, prescribed after proper evaluation, and appropriately documented and the documentation kept on file. There is no age restriction. Annual provider reviews and precise billing are essential for ongoing coverage. There is no age limitation. Billing codes are: HCPCS code B4100 (per ounce) and National Drug Code (NDC) when using CMS-1500 or 837P forms. There are no PA requirements and payment will be based on the lesser of the following: (1) The manufacturer’s suggested retail price (MSRP); or (2) The actual charge submitted.
